For good and valuable consideration from Women’s Prosperity Network, LLC., the receipt of which I hereby acknowledge, I hereby expressly grant to said Women’s Prosperity Network, LLC., and to its employees, agents and assigns, the right to film and photograph me and use my physical likeness (as the same may appear in any still camera photograph and/or motion picture film), in and in connection with the exhibition, theatrically, on television or otherwise, of any motion pictures in which the same may be used or incorporated, and also in advertising. I further give the said company the right to reproduce in any manner whatsoever any recordings made by said company of me including my voice and all instrumental, musical or other sound effects produced by me. 

This authorization will remain in effect for all future events as well until revoked in writing. To revoke this authorization simply email [email protected]
